A frequency record of observation is a record of repetitive behavior a child shows during a time period or throughtout the entir day. It can be used to monitor nondestructive behavior such as frequent bathroom breaks or it can be used to record behavior challenges such as biting and hitting. This form is special because it printed in carbon copy and so the parent and teacher can sign it and a copy can be given to the parents and the orginal can be kept on file at the school.
