In order to earn the 2 In-Service Hour Certificate the following must be completed:

-This form must be submitted before the training start or at the designated time during the presentation. 

-This form must be completed at the end or within 24 hours from the end off the training. 

-Must score at least 70% on the  post test at the end of the training.
